Bulk Donut Request Form

Please fill out this form if you are interested in ordering bulk cider donuts. Bulk cider donut pricing is $1.00 a donut and there is a minimum order of 3 dozen (36 donuts) to receive this pricing. Full payment is due at time of order (when we receive this form we will call you to confirm your request and collect payment) Your order will not be completed until payment is received.

Please note: all bulk donut requests must be placed at least 14 days prior to pick up date and are non-refundable.
